This service aims to assist you in restoring or maintaining your health and independence at home and assist you to avoid early admittance to hospital or residential care. We can assist with medication, wound care, hygiene and help with showering or dressing. A registered nurse will assess and consult with you to determine your needs for the type and frequency of nursing and\/or personal care services. To ensure you receive appropriate care for your clinical needs, the service will be closely monitored and reviewed by a registered nurse.
